2025 National Girls and Women in Sport Day Registration and Waiver
Oregon Athletics and Women In Flight are excited to invite you to our National Girls and Women in Sports Day Camp taking place Sunday, Janaury 19th prior to the Women’s Basketball vs Iowa game at Matthew Knight Arena!
This one-hour camp is FREE to all kids 6th grade and under that have a ticket to the January 19th Women’s Basketball game vs. Iowa and will be located in the Matthew Knight Arena Practice Courts near the East Entrance. Doors on the East Entrance will open at 12:30pm to allow for participants to enter the arena and arrive at the practice courts. This camp will showcase Oregon varsity women’s sports here at the University of Oregon, with representatives from available women’s teams there to interact with and teach their sport to the participants. Upon check-in, each participant will receive a free Women In Flight poster and takeaway gift (as supplies last) courtesy of Women In Flight and Toyota, our presenting sponsor for the National Girls and Women in Sports Day camp.
In order ease the check in process on Sunday, please complete the registration and waiver form below! Each child will need to complete the check in process and registration form PRIOR to coming to the clinic so reserve your spot today!
- Completing this form does not provide entrance into Matthew Knight Arena. A game ticket is needed in addition to the completion of this registration and waiver form.
- Once this form is completed and submitted, your child will be registered for the camp but does not guarantee access into the building.

2025 National Girls and Women in Sport Day Event Release and Waiver
In consideration of being permitted to participate in the National Girls and Women in Sport Event held at Matthew Knight Arena on January 19, 2025, you must first read and agree to the release and waiver agreement prior to entry.
In consideration of being permitted to participate in any way in the above-described activity (hereinafter called the “Activity”), I, for myself, my heirs, personal representatives and assigns, do hereby release, waive, discharge, and covenant not to sue the State of Oregon, the Board of Trustees of the University of Oregon, and the University of Oregon (collectively, hereafter called the “University”), their officers, employees, and agents from liability from any and all claims including the negligence of the University, its officers, employees and agents, resulting in personal injury, accidents or illnesses (including death), property loss, and damages arising from, but not limited to, participation in the Activity.
Assumption of Risks: Participation in the Activity carries with it certain inherent risks that cannot be eliminated regardless of the care taken to avoid injuries. The specific risks vary from one activity to another, but the risks range from (1) minor injuries such as scratches, bruises, and sprains (2) major injuries such as eye injury or loss of sight, joint or back injuries, heart attacks, and concussions to (3) catastrophic injuries including paralysis and death.
I have read the previous paragraphs and I know, understand, and appreciate these and other risks that are inherent in the Activity. I hereby assert that my participation in the Activity is voluntary and that I knowingly assume all such risks.
Indemnification and Hold Harmless: I also agree to INDEMNIFY, DEFEND, AND HOLD the University and its officers, employees, and agents HARMLESS from any and all claims, actions, suits, procedures, costs, expenses, damages and liabilities, including attorney’s fees brought as a result of my involvement in the Activity and to reimburse them for any such expenses incurred.
Medical Treatment Authorization: I understand that an emergency may develop which necessitates the administration of medical care. In the event of injury or illness, I authorize the University to secure appropriate treatment including the administration of an anesthetic and surgery. I understand that such treatment shall be solely at my expense. Notwithstanding this paragraph, I understand and agree that the University has no obligation to provide or seek out any medical treatment for me.
Acknowledgment of Understanding: I have read this waiver of liability, assumption of risk, and indemnity agreement, fully understand its terms, and understand that I am giving up substantial rights, including my right to sue. I acknowledge that I am signing the agreement freely and voluntarily, and intend by my signature to be a complete and unconditional release of all liability to the greatest extent allowed by law.
Model Release: I, the undersigned, irrevocably grant the University of Oregon permission to publish, republish, adapt, exhibit, reproduce, modify make derivative works, distribute, or display my name, image, voice, written testimony, and biographical information in connection with any university product or service. This permission applies to all markets and in any media or technology now known or hereafter developed. The university may exercise any of these rights itself or through any commercial or nonprofit successors, transferees, or licensees.
I waive any right to inspect or approve any work that bears my name, image, voice, written testimony, and biographical information.
