2005 West Regional Track & Field Championships

*To fill out a 2005 West Regional Track & Field Championships media credential application:
1. Click on the small printer icon, just underneath the headline.
2. Print out form and fill in the information completely
3. Fax it back to the University of Oregon Athletic Media Services office at 541/346-5449.
DEADLINE: All requests must be faxed no later than Thu., May 19, 2005 at 5 pm, and please call 541/346-5488 and confirm that it was received. (Please use a separate form for each media outlet.)
You will receive a confirmation of your credential request and further information for pick-up (must show valid ID), will call availability, and parking. Media parking is limited, and campus metered (8am-6 pm) and neighborhood on-street parking is also available.
